Beth yw ‘Car Linc Môn’?                                                               

Mae Car Linc Môn yn gynllun cludiant cymunedol cymdeithasol gwirfoddol ar Ynys Môn. Cynigir y gwasanaeth i bobl nad oes ganddynt unrhyw ffordd arall o wneud teithiau hanfodol.

Mae’r cynllun yn bennaf ar gyfer pobl sy’n byw mewn ardaloedd gwledig lle mae trafnidiaeth gyhoeddus yn brin, yn ogystal â’r bobl y mae eu hiechyd neu eu symudedd yn ei gwneud hi’n anodd defnyddio trafnidiaeth gyhoeddus mewn ardaloedd gwledig. Mae hefyd ar gyfer pobl sydd angen cymorth cwmnïaeth i fynd i glybiau, siopa neu eistedd mewn caffi gyda’i gilydd, neu fynd i gerdded.

Mae Car Linc Môn yn cynnig gwasanaeth cludiant hanfodol i bobl sydd angen:

  • Mynd i apwyntiadau ysbyty
  • Mynd i weld y meddyg, y deintydd neu optegydd
  • Mynd i Ganolfan Gofal Dydd
  • Casglu presgripsiwn
  • Gwneud ychydig o neges
  • Ymweld â pherthnasau neu ffrindiau
  • Mynd i ddigwyddiadau cymdeithasol ymgysylltu â’r gymuned (boreau coffi ac ati.)
  • Rhoi hwb i’w hyder drwy gael cwmnïaeth wrth ymweld â gardd neu leoliad arall
  • Cymryd rhan mewn gweithgareddau Presgripsiynu Cymdeithasol yn ôl cyfarwyddyd Meddyg Teulu.

Mae gyrwyr gwirfoddol yn amhrisiadwy o ran darparu gwasanaeth Car Linc Môn.

Dim ond gyda chymorth ein gyrwyr gwirfoddol ymroddedig sy’n defnyddio eu ceir eu hunain i gynnig y cludiant drws-i-ddrws hanfodol hwn y gallwn redeg gwasanaeth Car Linc Môn . Rydym yn cynnig hyfforddiant, cymorth, yn talu costau teithio ac mae ein gwirfoddolwyr yn dweud eu bod nhw’n cael teimlad boddhaus o wneud cyfraniad cadarnhaol o fewn eu cymunedau lleol.

Pwy sy’n trefnu ‘Car Linc Môn’?

Mae’r cynllun cludiant yn cael ei redeg yn ddyddiol o swyddfa Medrwn Môn yn Llangefni (dydd Llun i ddydd Gwener, 9 a.m. - 4 p.m.). Medrwn Môn yw Cyngor Gwirfoddol Sirol Ynys Môn. Mae Swyddog Cefnogi Linc Cymunedol yn gyfrifol am gymeradwyo’r holl geisiadau am gymorth/cludiant, trefnu bod gyrwyr gwirfoddol yn gwneud pob taith/bodloni cais am gymorth, a chynnig arweiniad os bydd unrhyw broblemau’n codi. 

Sut mae ‘Car Linc Môn’ yn gweithio?

Pan fydd cais yn ein cyrraedd gan unigolyn sydd angen cludiant/cymorth, bydd Swyddog Cefnogi Linc Cymunedol yn nodi’r manylion ac yn cadarnhau bod y cais yn ddilys. Yna, mae Swyddog Cefnogi Linc Cymunedol yn ffonio’r gyrwyr gwirfoddol i wirio pwy sydd ar gael. Mae angen rhybudd o o leiaf 3 diwrnod, a phan fydd yn bosibl, bydd teithiau/cymorth yn cael eu trefnu er mwyn cyfuno anghenion nifer o deithwyr. Bydd amcan o gostau a’r dull talu wedi cael eu trefnu cyn y daith, ynghyd ag unrhyw drefniadau eraill, e.e. amser dychwelyd, a oes angen cymar, ac a oes gan ddefnyddiwr y gwasanaeth Fathodyn Glas.

Bydd yr holl geisiadau'n cael eu hasesu ar eu teilyngdod. Gellir defnyddio’r gwasanaeth mwy nag unwaith yr wythnos, ond mae hynny’n dibynnu ar argaeledd ein gwirfoddolwyr.

What is ‘Car Linc Môn’?

Car Linc Môn is a voluntary social community transport scheme on Anglesey. The service is available to people who have no other means of making essential journeys.

The scheme is mainly for people living in rural areas where public transport services are scarce, but also for those whose health or mobility makes it difficult to use public transport  in rural areas and also need companionship support to attend clubs , go shopping or just sit in a cafe or go walking with. Car Linc Môn provides an essential transport service for people who need to:

  • Attend hospital appointments
  • Visit the doctor, dentist, optician
  • Attend a Day Care Centre
  • Collect a prescription
  • Do some shopping
  • Visit relatives or friends
  • Attend community engagement social events (coffeeornings etc)
  • Also for people who wish to build confidence through companionship whilst visiting a garden or other venue
  • Attend social prescription activities as directed by a GP

Our volunteer drivers 

Volunteer drivers are invaluable to delivering Car Linc Môn.

We can only run our Car Linc Môn service with the support of our dedicated volunteer drivers who use their own cars to provide this essential door-to-door transport. We provide training, support and mileage expenses, and our volunteers tell us they get a great sense of satisfaction from making a positive contribution in their local communities. Who organises ‘Car Linc Môn’?

The transport scheme is run on a daily basis from the Medrwn Môn office in Llangefni (Monday to Friday, 9 a.m. - 4 p.m.). The Community Linc Support Officer is responsible for approving all transport/ support  requests, arranging for volunteer drivers to make each journey / support and offering guidance should any problems arise. 

How does ‘Car Linc Môn’ work?

When an enquiry comes in from someone who needs transport / support, the Community  Linc Support Officer records the details and confirms that the request is valid. The Community Linc Support Officer then phones around to check availability of volunteer drivers. At least 3 days’ notice is required, and where possible, journeys/ support are arranged so that they combine the needs of a number of passengers. An estimate of cost, and the method of payment will have been arranged prior to the journey, along with any other arrangements, e.g. return time, whether an escort is required, and whether the service user has a blue badge.

All applications will be assessed on their merit. Access to the service is possible more than once a week, but only and depending on the availability of our volunteers.
